Upper E Extended Absence Instructions

At least one (1) week before your planned absence of three (3) or more days, complete this process for each student:

  1. Email notification to UpperElementary.4-6@chca-oh.org
  2. Include name and grade of student
  3. Include dates of absence with number of school days gone Include reason for absence

Note that Grade 5 and 6 homework and class assignments can be accessed through NetClassroom.

​Student Work Permit Procedure

  1. Print Work Permit Pre-Application form (.pdf) - Student, parents, & employer must sign
  2. Obtain physician approval using 1 of these ways:
    • Use Finalforms to confirm your physical on file with the Athletics Dept has not expired
  3. Present completed forms and a copy of student's birth certificate or driver's license to front desk in Miracle Commons and we will submit your permit online

Allow 24-48 hours after Step 3 is completed to obtain the actual permit.

Gr 12 Formal Senior Picture Info

4 possible 2019 Saturdays: 08/10/19 - 09/07/19 - 10/12/19 - 11/09/19

15 minute appointment between 10:00 am – 2:00 pm

Schedule formal Senior portrait

All senior pictures for the yearbook & composite displayed in Miracle Commons come from Dan Ledbetter Photography. Since the photo used is picked at the appointment, the photographer recommends a parent accompany their senior to the appointment.

Additional portraits may be taken at another time or with other photographers. Email questions to the Upper School 9-12 office.


Location: Dan Ledbetter Photography is located in the Landen Square Shopping Center, approximately 4 to 5 miles north of the school on Montgomery Road/22&3 (map).

Minimum Session Fee: $25.00 for inclusion in the yearbook and the CHCA alumni composite as well as comprehensive retouching; payable when you reserve your appointment. Prints may be ordered.


Dress Code:

  • Gentlemen: a dark suit jacket, white dress shirt and a dark tie
  • Ladies: wear or bring a tank top or short sleeve shirt as a formal black draping will be used so all the pictures are uniform. Please limit jewelry to small earrings and a pearl necklace. A pearl necklace is available to borrow for the photo session and a privacy top if what is worn does not work well with the drape.


Suggestions for a good portrait:

  • Face: Naturally, faces are the most important part of a portrait. Foundation make up should be blended at the jaw line for smooth transitions between face and neck. The camera picks up details in makeup very well and too much will not look good. Usually a natural, clean look works best.
  • Hairstyles: Simple and off-the-face photograph well. Hair falling down onto the sides of the face can create distracting shadows. Bangs that come down too low keep light from getting into the eyes. Men should have their hair cut not much sooner than one week prior to the scheduled appointment.
  • Glasses: You decide if you would like to wear your glasses while being photographed. If you choose to wear them, you may be asked to tip them up off your ears a bit to cut glare from studio lights. In some cases it's possible to obtain a frame set similar to your own from your eyewear specialist.

School Supply Lists: Upper 7 - Upper 8

Upper 9-12 students organize their class work however is best for them. A typical system is a binder per class, along with notebook paper and pens/pencils. Teachers will identify any other supplies on the first day of school.

Supplies for General School Use:
     Gr 9: 3 boxes of tissues, 3 packages of dry erase markers, and Lysol disinfecting wipes
     Gr 10: Minimum 3 pads of post-it notes (Size, shape, and color are up to you) + 2 boxes of tissues
     Gr 11: Minimum 2 boxes of tissues + 3 packages of blue, black or red pens
     Gr 12: Minimum 2 packages of dry erase markers + 2 packages of #2 pencils + 1 bottle of hand sanitizer


Info Update and Medical Forms:
  1. Respond to the secure, personalized link sent via email on 05/30/19 to update information online. This automatically sends the Authorization for Emergency Response (AER) form to your child’s nurse. (Invitations sent weekly during June and July for newly enrolling students. After 08/01/19, your Admissions Counselor will provide a paper copy of the AER form to complete and return.) All responses due 08/15/19.
  2. Consult the Medical and Other Forms webpage for any required print and return medical forms.

  • Register Online closes 09/17/19 at noon
  • Need ANY Over the Counter or Prescription Medications? Submit a K-12 School Medication Permission form signed by parent + doctor and any prescription meds that your child will need on the trip to the nurses' office by 09/03/19
  • A 2nd Parent Information session will be 08/28/19 at 6:00 p.m. in the Founder's Campus Gym
