Student Work Permit Procedure
- Print Work Permit Pre-Application form (.pdf) - Student, parents, & employer must sign
- Obtain physician approval using 1 of these ways:
- Have doctor complete the Physician's Certificate for Minor Work Permit (.pdf)
- Use Finalforms to confirm your physical on file with the Athletics Dept has not expired
- Present completed forms and a copy of student's birth certificate or driver's license to front desk in Miracle Commons and we will submit your permit online
Allow 24-48 hours after Step 3 is completed to obtain the actual permit.
Gr 12 Formal Senior Picture Info
4 possible 2020 Saturdays: 08/08/20 - 09/12/20 - 10/03/20 - 11/14/20
15 minute appointment between 10:00 am – 2:00 pm
Schedule formal Senior portrait
All senior pictures for the yearbook & composite displayed in Miracle Commons come from Dan Ledbetter Photography. Since the photo used is picked at the appointment, the photographer recommends a parent accompany their senior to the appointment.
Additional portraits may be taken at another time or with other photographers. Email questions to the Upper School 9-12 office.
Location: CHCA MSL Campus: Collaboration Center (Changed in 2020-2021)
Minimum Session Fee: $25.00 for inclusion in the yearbook and the CHCA alumni composite as well as comprehensive retouching; payable when you reserve your appointment. Prints may be ordered.
Dress Code:
- Gentlemen: a dark suit jacket, white dress shirt and a dark tie
- Ladies: wear or bring a tank top or short sleeve shirt as a formal black draping will be used so all the pictures are uniform. Please limit jewelry to small earrings and a pearl necklace. A pearl necklace is available to borrow for the photo session and a privacy top if what is worn does not work well with the drape.
Suggestions for a good portrait:
- Face: Naturally, faces are the most important part of a portrait. Foundation make up should be blended at the jaw line for smooth transitions between face and neck. The camera picks up details in makeup very well and too much will not look good. Usually a natural, clean look works best.
- Hairstyles: Simple and off-the-face photograph well. Hair falling down onto the sides of the face can create distracting shadows. Bangs that come down too low keep light from getting into the eyes. Men should have their hair cut not much sooner than one week prior to the scheduled appointment.
- Glasses: You decide if you would like to wear your glasses while being photographed. If you choose to wear them, you may be asked to tip them up off your ears a bit to cut glare from studio lights. In some cases it's possible to obtain a frame set similar to your own from your eyewear specialist.

08/12/20 Update for Gr 7-8: Since COVID prevents us from sharing supplies, your student should also bring a small personal container that includes: scissors, glue stick, small set of colored pencils, a small basic calculator

Upper 9-12 students organize their class work however is best for them. A typical system is a binder per class, along with notebook paper and pens/pencils. Teachers will identify any other supplies on the first day of school.
Supplies for General School Use: (If you have trouble finding an item on your student’s supply list, simply substitute with an item from another list or additional items from your student’s list.)
- Gr 9: Minimum 3 boxes of tissues + 2 packages of dry erase markers + 2 containers Lysol disinfecting wipes + 1 bottle hand sanitizer
- Gr 10: Minimum 3 boxes of tissues + 3 pads of post-it notes (Size, shape, and color are up to you) + 1 container Lysol disinfecting wipes + 2 bottles hand sanitizer
- Gr 11: Minimum 3 boxes of tissues + 3 packages of blue, black or red pens + 2 containers Lysol disinfecting wipes + 1 bottle hand sanitizer

Info Update and Medical Forms:
- Respond to the secure, personalized link emailed by 06/05/20. This automatically sends the Authorization for Emergency Response (AER) form to your child’s nurse. (Invitations sent weekly during June and July for newly enrolling students. After 08/01/20, your Admissions Counselor will provide a paper copy of the AER form to complete and return.) All responses due 08/12/20.
