Returning Students

*All re-enrolling students must complete and return medical forms to your building nurse by August. Form requirements and need for physician's signature vary based on your child's grade, unique medical history, athletic involvement, and travel. Forms are posted in March for the following school year.

*The International Student Department will work directly with re-enrolling ISP students to complete their requirements.

NEW North Campus Students (PK - Gr 12)

CHCA operates under a Board of Trustees who are elected by members of the CHCA Association. Membership is limited to parents of enrolled students and the acting Board. To become a member and gain voting privileges, complete this form when your first child enrolls at CHCA.

CPS Private/Parochial School form

Student Billing Information

For the 2024–25 school year, we are transitioning to a new tuition payment system, FACTS Tuition Management. Details on payment plans and registration will be communicated beginning mid-May.

Enrollment in FACTS is required for monthly, quarterly, annual, and semi-annual tuition payment options. Find more information here.

Please note FACTS can be accessed through the Student Billing Homepage button in the Veracross Parent Portal, along with previous invoices. Invoices will be available through FACTS in mid-May or within two weeks of enrollment for students enrolling after that time.
