In order to participate on any team at CHCA, players must have three forms on file by the first day of practice: an OHSAA sports physical, a CHCA participation form and a CHCA transportation release.  For their own safety, no athlete will be permitted to participate in practices or contests without these forms on file.

Each form is available online on the Parent Page of this site.  Paper copies are available in the Athletic Office.

In addition, there is an Athletic Participation Fee required to participate in any 7-12th grade sport at CHCA.
