Participation fee: $225 first sport, $175 second sport, $75 third sport
Team Gear (purchased through Varsity Spirit): Uniform: $220, Polar Fleece: $90, Fleece Headband: $21, Shoes: $59, Custom Jacket: $64, Bow: $15, Clear Rain Jacket: $13, Poms: $28, Camp Shirts/Spiritwear:  $45, Cheer Bag: $28 = $583
Required individual gear (not purchased through school): None
Spirit wear: available for purchase through Eagle Store Spirit Pack: Cheerleading T-shirts, sweats, hats, bags, car magnets, etc. (cost varies)
Expected Parent Involvement: We are always excited to have team moms assist throughout the year with different activities. We like to join together to make the cheer experience at CHCA a family between the parents, athletes, and coaches.

Fall 2016 Three Step Registration Process

How To Sign Up:

(Fall 2016)
  1. Register and schedule payment online in UltraCamp to join team roster
  2. Complete athletic eForms online in FinalForms  (This FinalForms Parent Playbook will help you get started.)
  3. Return paper OHSAA Physical pages 3 & 4 (requires doctor signature) to the Athletic Dropbox in the trophy hallway outside the Upper 9-12 gym.  Do NOT return forms to the front desk at any building or directly to your coaches.  Two ways to print this:
    • Print a pre-completed version directly from FinalForms
    • Print a blank version from this PDF
Cancellation Policy:  If you or your student-athlete decides to leave the team after they are registered in UltraCamp, you are responsible log back in to your UltraCamp account and cancel the reservation. You must cancel your registration before the first competition otherwise you will be charged the full amount and no refund will be given. Reporting this change to the coach, the Athletic Office or Programs@chca-oh.org does not cancel your registration or eliminate your scheduled payment and you will still be charged the full amount and no refund will be given.

08/26/16 - Payment automatically withdrawn from account specified upon registration
